You and your family are very important members of this team. As team members you will be involved in all aspects of your rehab program.

Physiatrist

A physiatrist (a doctor trained in rehabilitation) will be coordinating care at the rehab center and offering medical direction to the rehab team. This physician may consult with your primary care physician, as well as other medical specialists if the need arises.


Rehabilitation Nurse

The rehab nursing staff is available 24 hours a day, seven days a week to help you build upon skills learned in therapy and to assist in transferring these skills into daily routine. Nursing efforts focus on skin care, bowel and bladder training, medication routines, nutrition plans, health education and overall, health maintenance. The goal of the nursing staff is to assist you and your family to become as independent as possible, while assuring that all health care needs are met.


Social Worker

The social worker will meet with you and your family to obtain a social history. Throughout your stay, the social worker will work with the other team members to assist you with information related to your progress, goals and discharge. Planning for your discharge can be an overwhelming task. The knowledge and expertise of the social worker can be invaluable in gaining information about community resources. Occasionally, patients and/or family members may face difficult decisions about care. The social worker is available to provide guidance and consultation when difficult decisions arise.


Physical Therapist (PT)

The physical therapist is concerned about your ability to move from one place to another and the amount of movement and function of your body. The PT will evaluate your physical status to determine your capabilities and limitations, and then develop treatment goals with a time line for accomplishing these goals along with you. The PT will also recommend any equipment (braces, canes, wheelchairs, etc.) that will foster your functional ability and will instruct you and your family on their use and care.


Occupational Therapist (OT)

The occupational therapist helps you lead as independent, productive and satisfying life as possible. The OT assesses and analyzes your physical, social and cognitive skills and works with you to discover ways to increase your independence. OT uses everyday activities and assistive devices, when appropriate, to increase your independence in day-to-day living activities as basic as bathing and dressing or as complex as returning to work.


Speech Language Pathologist (ST)

If you are having difficulty communicating with others you will be evaluated by the speech language pathologist. You may attend individual therapy sessions designed to help improve your ability to speak or understand others, depending on your needs. Problems with swallowing may also be evaluated and therapy to help swallowing will be completed. If you are having difficulty with memory, attention span or problem solving, the ST will also assist in those areas.


Therapeutic Recreation Specialist (TR)

A therapeutic recreation specialist will help you and your family identify meaningful leisure time activities. You will be encouraged to participate in individual and/or group activities which include socialization groups, orientation groups, arts and crafts, games, holiday parties and special events. The purpose of these activities is to concentrate on skill development, constructive use of leisure time, community reintegration and social interaction.


Psychologist

Disorders that affect the nervous system, such as stroke or head injury, may lead to changes in the way a person behaves or thinks. A psychologist/neuropsychologist is available to assist you and your family in adjusting to the changes in lifestyle that can occur following this type of illness. The psychologist may also have you take part in psychological testing to better assess your current abilities, interests, personality, and thinking skills.


Case Manager

The Case Manager will assess you for your rehab needs and will help you to understand our program. The Case Manager will communicate with payors and other health care professionals in determining your needs. While on our unit, the Case Manager will assist in keeping your primary physician up to date on your progress and the potential discharge plans.



Additional Team Members
Registered Dietician

Adequate nutrition is an important part of your recovery process. Menus are distributed each morning to permit you to select your own food choices in accordance with your prescribed diet. A registered dietician is available to discuss your diet and any nutritional concerns.


Chaplain

Provides pastoral care and counseling to help you and your family deal with the spiritual impact of your condition.


Respiratory Therapist

Uses advanced diagnostic and therapeutic equipment to thoroughly assess and treat your ability to breathe. The respiratory therapist tailors a specific rehab program to fit your needs.


Prosthetist/Orthotist

fabricates, fits, and repairs adaptive devices such as artificial limbs (prostheses) and orthopedic braces (orthoses).
