Your core team will consist of a number of professionals depending on your program and goals.

- Programs are individualized for each person served and provide or arrange for the following services including:
- Gait/Mobility and Transfer Training
- Activities of Daily Living (ADL) Retraining
- Muscle and Movement Re-education
- Cognitive/Perceptual Motor Retraining
- Treatment of Communication Disorders
- Vocational Rehabilitation
- DME (Durable Medical Equipment)
- Driver evaluation and/or education
- Audiological (Hearing) Services
- Chemical dependency counseling
- Diagnostic radiology
- Dysphagia (Swallowing) program
- Laboratory
- Rehabilitation Engineering
- Patient and Family Education
- Supportive Services/Groups
 Team Conferences
Your professional team will meet at least once a week to discuss your development. Issues and concerns are addressed as well as future planning. After the meeting a core member will review discussion and plans with you.
 Family Conferences
Your discharge planner will meet with you and your family to openly discuss progress and plans that were reviewed in the team conference.
The scope of this service relates to your functional limitations and amount of support to be expected from family members.
 Physician Team Work
Physician coverage is available for medical needs 7 days per week, 24 hours per day. This is normally provided from a physician other that your physiatrist. However, your physiatrist has final authority and responsibility for your program in partnership with the rehab team.
 Discharge Planning
We will discuss with you your desired ultimate living situation, the accessibility of your current living arrangements, and your family's capability to provide support. From this information a goal is designed to make you as independent as possible.
You may be requested to perform a therapeutic leave evaluation. This may require you spend a few hours in your home with family, negotiating stairs, bathrooms, etc.
Final discharge alternatives may include:
- Home
- Home With Outpatient Therapy
- Assistive Living Arrangements
- Transitional Living Facilities
- Long-Term Care
