PHA Health Benefit Plan

This is an excellent Health Plan for many members of the Physician Hospital Alliance.  I hope you will consider offering the Health Plan in your office.   

A benefit plan summary is available here.  As you will see there are two options available - a Preferred Provider Organization (PPO) and a Health Savings Account (HSA).  Their respective premiums are also included.  There are three brief forms that will need to be completed and returned to me if you wish to participate for 2010.  The forms are:

I would be happy to visit your office to answer questions and deliver the necessary forms.  This Health Plan is not available through your regular insurance agent or broker and is only available to PHA members.  It is a self-funded program with group rates.  Each physicians office will be treated as part of the group and offered the group rate, so there will be no individual underwriting.  A sponsoring physician must participate for the plan to be available to the staff.  The Code of Regulations for the PHA Group Benefit Association are available here.

 The requirements for participating will include:  

• Enrollment is during the annual enrollment period unless there are qualifying events
• The plan must be offered to all full time employees within the physician practice
• Full time employees within the participating practice must enroll or prove they have
  insurance through a spouse 
• Physician/group must contribute at least 50% of the expected premium of a single plan    
  for each employee who enrolls
• No other health plan can be offered in the practice
• If the practice drops coverage no one within that practice will be eligible for
  participation for a three year period 
• The practice will be billed monthly
• The practice will be responsible for notifying PHA of eligibility and enrollment changes
• The practice will collect premiums from its own employees and reimburse PHA for all
  eligible employees on a monthly basis
